Requirements:
Capstone Hospice is currently seeking a Part-Time On-Call Hospice RN to join our on-call team of dedicated nurses.
This position will work an assigned shift.
The On Call Hospice RN is a part of the On Call team who provides virtual care to patients via telephone, at times in-person visits may be required.
The ideal candidate for this role is one who has a passion for hospice care, is empathetic, warm and compassionate.
In addition to being a great listener, must be a strong communicator and have the sense to know when to ask more or less questions and be respectful and professional in our clinical care telephone demeanor.
This position requires someone that can demonstrate and exercise independent and sound judgment to assist patients with complex and diverse issues effectively and efficiently.
This is a weekend shift working Saturday and Sunday, 8am - 5pm, considering candidates that are available to work every weekend or every other weekend.
Essential responsibilities of the role:
Triage calls to the appropriate level of care.
Providing advice on symptom management using pharmacological and non-pharmacological measures.
Answering any questions and providing education over the phone regarding patient's condition and placing courtesy calls to follow up on patients post intervention.
Expectations of assisting medication orders, DME and faxing documents/orders as needed.
Submit a report of all calls (incoming and outgoing) at the end of shift as well as maintaining an activity log for all in-person visits that might have occurred during the shift.
Observe signs and symptoms and reports to the physician and other team members any unexpected changes in the patient's physical or emotional condition.
Organizes work schedule and utilizes time management to be able to attend all required meetings.
Develops and re-evaluates the patient/family care plan to meet their needs and maintain continuity of care.
Documents visits in electronic medical record system (Wellsky Consolo) Performs specific nursing procedures as needed (e.
g.
, treatments, management of symptoms) following doctor's orders.
Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient's physical and emotional conditions.
Follows the policies and procedures of Hospice.
Observes confidentiality and safeguards all patient-related information in compliance with the HIPAA regulations.
Complies with infection control policies and protocols.
Candidates must meet the following qualifications and experience to be considered:
Graduate from an accredited school of nursing.
Currently licensed as a registered nurse to practice in the state of Georgia.
Minimum of 2 years as a registered nurse.
Active patient contact within the past three years, preferred.
Must provide evidence of annual TB test and other state required tests or examinations.
Must have current valid driver's license, auto liability insurance and your own reliable transportation.
To be successful in this role you should have the following knowledge & skills:
Ability to work independently.
Make accurate and at times quick judgements.
Ability to respond appropriately to crisis outside of a hospital setting.
Acceptance and adaptability to different social, racial, cultural and religious modes.
Have completed or the ability to complete a designated Hospice training program.
Position Factors:
Physical Requirements:
Requires considerable physical effort when in-person visits do occur, including, kneeling, squatting, reaching, twisting, climbing, walking, exposure and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient.
Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position.
Mental Requirements:
Must be able to work independently, make judgements based on assessments and data available and act accordingly.
Must be flexible, innovative, and possess good interpersonal skills.
Must be able to cope with mental and emotional stress and demonstrate emotional stability.
Working Conditions:
Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids, excrements, adverse environmental conditions and hazardous materials.

Estimated Salary: $20 to $28 per hour based on qualifications.
