Requirements:
Capstone Hospice is in search of a Clinical Admissions Supervisor. This is a full-time supervisory position that has direct reports.
This position will report to the Director of Clinical Services and will work out of our Peachtree Corners office, This is a salaried-exempt position.
Candidate must have the following qualifications to be considered:
Graduate from an accredited school of nursing.
Licensed as a registered nurse to practice in the state of Georgia.
Minimum 5 years experience as an RN.
Minimum 2 years of hospice experience.
Minimum 2 years in a supervisory or manager position, with direct reports.
Knowledge of Medicare, Medicaid and third-party reimbursement requirements for hospice.
Extensive knowledge of the regulatory guidelines governing hospice admissions.
Candidates must possess the following traits to be successful in this role:
Ability to work independently and within a collaborative team.
Ability to deal effectively with high levels of stress.
Ability to work with different disciplines within Capstone Hospice.
Strong attention to detail.
Strong organizational skills.
Essential Job Duties:
Clinically assess hospice referrals (either on paper and/or in person) for hospice appropriateness and admission to the hospice program.
Ensures approved hospice referral are visited promptly by an Admission Nurse.
Monitors time management of Admission Nurses and ensures efficient utilization of admissions personnel.
Provides ongoing training surrounding admission criteria to all members of the department.
Assists in gathering appropriate information to assist the clinical team in scheduling an evaluation visit.
Completes evaluation visits if Admission Nurses are occupied or otherwise not available.
Completes admission consent forms with families when necessary or appropriate.
Communicates (oral and written) pertinent information to the appropriate care team.
Trains new Admissions Nurses in the admissions process.
Obtain verbal order for patients when necessary.
Attends weekly sales meetings.
Communicates and follows up with the Intake Supervisor regarding pending admissions.
Assists with entering referral information into hospice EMR.
Enters non-admit information into EMR with details in coordination notes. Communicates non-admit status to referral sources and or/ Clinical Director with the reason for non-admission.
Assist with verification of benefits.
Communicate customer service issues to the team as appropriate.
Educate prospective patients, families, physicians and hospitals regarding hospice services.
Serves as the liaison between clinical and marketing staff.
Participate in on-call rotation during work hours on company holidays.
Remains willing to serve in any capacity necessary (as long as within his/her professional scope of practice) to support the hospice mission.

Physical requirements:
Requires considerable physical effort most of the day to include kneeling, squatting, reaching, twisting, walking, exposure to to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100
pound patient. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential function at a level required to meet the essential duties of the position. Must provide evidence of annual TB test and other state- required tests or examinations.
Mental requirements:
Must be able to work independently, make judgements based on assessment and data available and act accordingly. Must be flexible, innovate and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.
Working conditions:
Be able to tolerate exposure to elements including, but not limited too, odors, blood, body fluids and excrement's, adverse environmental conditions and possible hazardous materials. Be able to work in an office that may requirement sitting, walking and standing at times.
Risk exposure to bloodborne pathogens: Low Risk
Transportation:
Must have a current valid driver's license, auto liability, insurance and reliable transportation.
This a full-time salaried exempt position that is eligible for the Capstone Hospice benefits. Benefits include: paid time off, holidays off, health and wellness benefits (medical, dental, vision, life and disability insurance).Estimated Salary: $20 to $28 per hour based on qualifications.
